Certificate Request Forms
If you are in need of a certificate of Baptism, First Holy Communion or Confirmation; a letter of Eligibility to be a Godparent or Confirmation Sponsor, or need a letter stating you attended our Faith Formation or RCIA programs, we request that you click the link below, print out the form, complete and deliver it to the Parish Office in person, by email or by FAX (407.322.1131). Requests received by Wednesday will be available on Friday.  Should you have any questions, please contact the parish office at 407.322.3795 .

Godparents/Sponsor "Affirmation of Faith and Morals" Form
It is the very ancient custom of the Church that at the celebration of the Sacrament of Baptism or Confirmation, a godparent(s)/sponsor is present. Click on the link below to download or print the form that Godparents/Sponsors need to submit to confirm their eligibility.
